Where the story starts.

Healthcare organisations frequently operate through fragmented systems where registration, appointments, consultations, prescriptions, diagnostics, pharmacy and payments each work independently. For a patient this means the same history is retold at every step; for clinicians and administrators it means information is moved manually between departments. CodeFacts set out to bring these workflows into one connected digital ecosystem — not merely to digitise records, but to build an architecture capable of eventually supporting intelligent clinical workflows.

The challenge

The hospital ran as disconnected steps. The patient needed a single, continuous record.

The traditional hospital workflow ran as a chain of independent transactions — registration, appointments, consultation history, prescriptions, pharmacy, laboratory, billing, follow-up and notifications — with each step isolated from the next. Doctors manually reviewed previous consultations, admin teams shuttled information between departments, and engagement after a patient left was limited. What was needed was a single longitudinal patient context in place of scattered, independent transactions.
